Student Self‐Declaration Web-Form

The Department of Anatomy and Cell Biology is committed to supporting students who need to defer a midterm exam or other in-course assessments, or who have missed class, when no official supporting documentation is available.

In line with the Quebec government’s directive to reduce the strain on an already taxed healthcare system, students are no longer required to submit a medical note for incidental illnesses lasting fewer than five days.

However, supporting documentation is still required in the following cases:

  • For illness-related absences of five (5) or more days

  • For all non-medical deferral or absence requests

Valid documentation must be submitted to the Course Coordinator within five (5) working days of the original assessment or missed class date.

1)Completion and submission of this form does NOT imply approval. The decision to approve or deny a request rests with the Course Coordinator. 2) Making a false statement on this form is considered a violation of the Code of Student Conduct and Disciplinary Procedures, Section 19, Misrepresentation of Facts (a) “It shall be an offence to misrepresent material facts to another for the purpose of gaining admission to the University or gaining academic advantage or credit” and will be reported as an academic offence to a Disciplinary Officer. 3) All submitted forms will also be kept on file at the Anatomy & Cell Biology department office for future verification.
